Replacing Your Medicare CardIf you need your Medicare card replaced, call the National Social Security Hotline at (800) 772-1213.

You can also visit your local Social Security office, or make the request online at SSA.gov. Be ready with your Social Security number, date of birth and your name as it appears on your most recent Social Security card.

If you get Medicare benefits through a Medicare Advantage Plan such as an HMO, PPO, or PDP, you will need to get in contact with your plan to get your plan card replaced.
